IF YOU ARE INTERESTED IN ADOPTING A PUPPY
OR DOG PLEASE COMPLETE THIS QUESTIONNAIRE.

The following questionnaire is to gather information from you to help us place the appropriate puppy in your home. Please be advised that as the breeder, we reserve the right to keep one or more of our puppies out of any litter for our personal goals for our kennel. Our goal is to have a final decision on our pick(s) of the litter by the age of eight weeks, however, in some cases it may take longer.

Decisions on placement will be made according to the information gathered from this questionnaire, the desires that you have made known to us, temperament of the puppy, quality of the puppy, matching the right puppy to the right home, any other factors deemed relevant by us, and possibly a home inspection by us or someone of our choosing. Puppies are priced according to their quality as determined at eight weeks of age, or sooner if possible. It is agreed that placement of all puppies shall be at our discretion and in the best interest of the puppy.

If you have made a deposit and a puppy does not meet the needs that you have expressed or we feel that the puppy is not a proper fit for your home, you shall receive a full refund. If both parties agree, the
deposit may be transferred to a future litter.
